Your initial Appointment

Please bring:

• A referral letter from your GP or specialist

• Any relevant test results including scans, x-rays and blood tests

• Patient Registration Form

• You can send all of the paperwork above using the form at the bottom of this page

• Your Medicare card and any concession cards

Referrals

• A valid referral from your GP or specialist is required in order to claim a Medicare rebate.

• If you forget to bring your referral on the day, then just send it to us afterwards making sure it is dated before or on the same day as your appointment.

• Once we receive the referral, we will process your Medicare claim and the rebate will be paid directly into your nominated bank account.

• GP referrals last for 12 months and specialist referrals last for 3 months

Payments

• Consultations must be paid in full on the day of the appointment before any rebate can be applied.

• We are unable to bulk bill or delay payment if a referral is missing and the full fee is applied.:
